Prior to admission in a hospital/clinic the patient (Employee) will simply complete a Claim Notification Form (available with the employer) and submit it to Employer along with consulting Doctor's Advice Note. The Employer will send it to Pragati Life to obtain a letter of authorization for admission and advise the patient accordingly.

The patient shall submit the claim through the Employer in the prescribed form along with the original supporting documents for reimbursement of expenses.

In case of emergency, prior authorization is not necessary but Pragati Life should be notified through the Employer within 48 hours of admission.
